Acknowledgement, Agreement and Declaration Relating to Coronavirus/Covid-19

​1.     I acknowledge and agree that each member or guest who comes onto the tennis courts and tennis facility premises used by the Mono Tennis Club is responsible for taking their own precautions to avoid contracting COVID-19 and at a minimum practice precautions in accordance with the guidelines issued by Wellington-Dufferin-Guelph Public Health (wdgpublichealth.ca).
2.     I acknowledge that persons other than Mono Tennis Club members are permitted to use the Town of Mono tennis courts and tennis facility premises and that the Mono Tennis Club has no duty or opportunity to regulate their condition or behaviours as it relates to COVID-19.
3.     I acknowledge and agree that no member or group of members of the Mono Tennis Club, including but not limited to its Officers or Board, have the duty or opportunity to take precautions to avoid the contracting of COVID-19 by others, including but not limited to cleaning or sanitizing any part of the Town of Mono tennis courts or tennis facility premises.
4.     I acknowledge the highly contagious nature of COVID-19 and I voluntarily assume the risk that I (or my child, if participant is a minor/or the person I am the tutor or legal guardian of) could be exposed or infected by COVID-19 by participating in the Mono Tennis Club’s activities. Being exposed or infected by COVID-19 may particularly lead to injuries, diseases or other illnesses.
5.     I declare that I (or my child, if participant is a minor/or the person I am the tutor or legal guardian of) am participating voluntarily in the Mono Tennis Club’s activities.
6.     I declare that neither I (or my child, if participant is a minor/or the person I am the tutor or legal guardian of) nor anyone in my household, have experienced flu-like symptoms in the last 5 days (including fever, cough, etc.).
7.     I declare that if I (or my child, if participant is a minor/or the person I am the tutor or legal guardian of) experience, or if anyone in my household experiences any flu-like symptoms after submitting this declaration, I (or my child, if participant is a minor/ or the person I am the tutor or legal guardian of) will not attend any of the Mono Tennis Club’s activities, programs or services until at least 5 days have passed since those symptoms were last experienced.
8.     I declare that neither I (or my child, if participant is a minor/or the person I am the tutor or legal guardian of), nor any member of my household have travelled to or had a lay-over in any country outside Canada in the past 5 days. If I (or my child, if participant is a minor/or the person I am the tutor or legal guardian of) travel, or if anyone in my household travels, outside Canada after submitting this declaration, I (or my child, if participant is a minor/or the person I am the tutor or legal guardian of) will not attend any of Mono Tennis Club’s activities, programs or services until at least 5 days have passed since the date of return.
9.     I (or my child, if participant is a minor/ or the person I am the tutor or legal guardian of) agree to the requirements and recommendations of national, provincial and local Public Health and other governmental authorities and to those special safety regulations put in place by Mono Tennis Club as it pertains to the Coronavirus/Covid-19 and to adopt all necessary measures to those effects.
 
This document will remain in effect for the duration of my membership.
